Referral forms can be downloaded in the link below. Please also forward any available ultrasound results.
Patients are seen at either of our office locations in Kitchener or Guelph. We assess patients from Waterloo, Kitchener, Cambridge, Guelph, London, Milton, Burlington, and surrounding areas. Consultations include an on-site ultrasound performed by a Vascular surgeon followed by a discussion of treatment options.
